Insurance

Due to the recent changes in our national health care system, it is nearly impossible to verify what procedures are covered under every patients' plan. Please call your insurance company prior to coming to your appointment to verify what is covered and if you have a deductible and/or co-pay. Insurance companies require us to collect co-pay and deductible amounts on the day of service. We will do our best to promptly file claims to your insurance for you.

Your insurance may have different coverage for certain procedures. The following codes are for common procedures done in our office that might be helpful to verify prior to your appointment:

• 95004 - allergy testing
• 95165 - allergy serum
• 95115 - allergy injections
